Why measure care trajectory management?

Care trajectory management is a relatively new concept, which refers to the safety critical, but largely invisible, nursing practices that hold together patient care.

Patient care is complex and healthcare systems are inherently turbulent.

Nurses make multiple interventions to ensure that patients receive the right care, in the right place, at the right time, by the right person with the right outcome.

    TRACT generates data on the different sources of care trajectory complexity on a ward or unit, providing evidence of where changes and interventions can be made to help reduce the organisational workload.

  • TRACT builds on research which for the first time made visible the organisational elements of nursing work (Allen, 2015). The development of TRACT was funded by awards from the UK Economic Social Research Council (ESRC) designed to translate the findings of social sciences research for societal impact.

  • TRACT was developed by a team led by Davina Allen, Professor of Nursing, Cardiff University. The development process involved nurses from Cardiff & Vale University Health Board representing different specialities and different levels of experience, international nurse academics, and nurse policy leaders.

  • A range of tools exist for the purposes of measuring nursing work. These focus on patient acuity or dependency. No tools exist that are explicitly designed to assess the organisational aspects of patient care. This means that currently nursing work is only partially recorded, and that workload modelling fails to capture the full complexity and volume of nursing work.

  • A range of nurse staffing tools and methodologies are used internationally. TRACT has been designed to complement a triangulated approach to workforce planning and work alongside existing tools. A triangulated approach involves using several tools or methodologies to inform professional judgements in nurse staffing decisions. TRACT is an important addition to the nursing workforce planning toolkit.

  • TRACT includes an integrated TRACT Planner to enable seamless patient assessment and planning.
